Comments:
An unannounced renewal inspection was conducted today from 11:30am-1:00pm. There were 9 children (21 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, working on crafts, watching videos, and eating lunch. Today's lunch consisted of turkey, pasta, carrots, bread, bananas and apples. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-700-A
Description: Based on provider interviews it was determined that a child had two medications on site without a medication authorization form. Evidence: Child#6 has two long term medications on-site without a medication authorization form completed and signed by the parent and the child's physician.

Plan of Correction: We will ask the parents to bring an authorization form signed by the parent and the pediatrician.

Standard #: 22VAC40-111-120-B
Description: Based on record review, it was determined that the center did not maintain complete staff records.

Evidence:
The records of Staff#2 and Staff#3 did not contain two verified references as to character and reputation as well as competency prior to their first day of employment.

Plan of Correction: We will request references for both staff members.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: The records of Child#2 and Child#3 do not contain their medical insurance policy number.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain additional immunizations once every six months for children under the age of two years.

Evidence:
Child#1's record contains an immunization record that was last updated on 3/28/2018. Child#5's record contains an immunization record that was last updated on 8/2/2018.

Plan of Correction: Requested during the inspection.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, the family day home did not obtain a fingerprint based background check for each staff member prior to their first day of employment. Evidence: The records of Staff#2 and Staff#3 do not contain fingerprint check results prior to their first day of employment. The first day of employment of Staff#2 was documented as 6/17/2019. The first day of employment of Staff#3 was documented as 6/24/2019.

Plan of Correction: They have an appointment scheduled to get the fingerprint checks done.
