Evidence: Child#6 has two long term medications on-site without a medication authorization form completed and signed by the parent and the child's physician. Plan of Correction: We will ask the parents to bring an authorization form signed by the parent and the pediatrician.
Standard #: 22VAC40-111-120-B Description: Based on record review, it was determined that the center did not maintain complete staff records.
Evidence:
The records of Staff#2 and Staff#3 did not contain two verified references as to character and reputation as well as competency prior to their first day of employment.Plan of Correction: We will request references for both staff members.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: The records of Child#2 and Child#3 do not contain their medical insurance policy number. Plan of Correction: Corrected during the inspection.

Standard #: 63.2-1720.1-B-2 Description: Based on record review, the family day home did not obtain a fingerprint based background check for each staff member prior to their first day of employment. Evidence: The records of Staff#2 and Staff#3 do not contain fingerprint check results prior to their first day of employment. The first day of employment of Staff#2 was documented as 6/17/2019. The first day of employment of Staff#3 was documented as 6/24/2019. Plan of Correction: They have an appointment scheduled to get the fingerprint checks done.
