Discussed supervision of sleeping infants, behavioral guidance and staff references.
An unannounced renewal inspection was conducted beginning at 11:15 a.m. and ending at 1:30 p.m. with the provider and two assistants. There were 11 children in care ranging in age from 11 months to 6 years of age for a total of 26 points. Older children were observed during learning television time. Younger children were observed during diapering. After diapering, younger children participated in a craft. Children prepared for lunch and hand washing procedures were observed. Lunch today was whole wheat buns with sloppy joe, carrots, kiwi and milk. Five children's records and three staff records were reviewed. The monthly menu was posted. Attendance record was reviewed. Standard #: 22VAC40-111-120-B Description: Based on review of staff records, not all records included required information. Evidence: Staff #3, hired 6/14/2019, did not have documentation of two references that included date of contact, results, and the signature of the person's taking the reference. Plan of Correction: This information was updated during the inspection.
Standard #: 22VAC40-111-560-A Description: Based on observation, direct care and supervision including awareness of and responsibility for each child in care, including being near enough to intervene if needed was not maintained. Evidence: In the infant room off of the common area with the couch, two infants age 11 months and 16 months were left in the room with the gate closed while Staff #2 went to the front infant room with another infant for a diaper change. The other infant room is approximately 20 feet away. Two other staff were present in the kitchen area that is approximately 15 feet from the occupied room. No staff was near enough to ensure awareness of each child and intervention if necessary. Plan of Correction: In the future, any need to leave a room will be communicated to staff to assist.
