The inspector conducted an announced monitoring inspection on July 19, 2019 from approximately 12:15 PM to 1:00 PM to examine the areas of noncompliance identified at the initial inspection. The center's policies and procedures were reviewed, observations were conducted and records were reviewed. There were repeat violations cited. Standard #: 22VAC40-111-800-A Description: Based on observation and interview, the provider did not ensure the family day home shall have a written emergency preparedness and response plan. Evidence: The licensing inspector observed that the emergency response plan which contained information for emergency responses at a previously operated child day center. The provider stated no emergency response plan was available to address responses to emergency situations specific to the family day home. Plan of Correction: The provider will use the licensing template to create an emergency preparedness plan specific to the family day home.
Standard #: 22VAC40-111-330-A Description: Based on observation and interview, the provider did not ensure that there was a landline telephone available, operable, and accessible that does not require electricity to operate. Evidence: The licensing inspector observed no landline telephone in the family day home. The provider stated the home did not have a landline phone that does not require electricity to operate. Plan of Correction: The provider reports the phone service company has advised that their phone service is strictly through the modem, however they will provide a back up battery for the modem that will work during a power failure.
Standard #: 22VAC40-111-430-A Description: Based on observation, the provider did not ensure that children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs. Evidence: The licensing inspector observed no protective barrier to prevent access to the stairs. Plan of Correction: The provider will purchase a metal stair gate for the bottom of the stairs.
