Comments:

An unannounced renewal inspection was conducted today 7/22/2019. The inspection began at 11:45am and concluded at 1:30 PM. There were twelve children in care with three caregivers and a total of thirty one points. During the inspection infant care was observed, lunch time was observed, free play outside, hand washing was observed, five out of twelve children's records were reviewed, diapering was observed the licensee's record was reviewed, one assistant's records were reviewed, emergency supplies were viewed, and some policies and procedures were reviewed.

Description:	Based on review of five children's records, the provider failed to ensure that all records were complete. Evidence: 1. Child # 2 was missing the fathers employer's address. 2. Child # 5 was missing the fathers employer's address and phone number.
Plan of Correction:	The provider will have the parent to complete the missing information ASAP.
