A Subsidy Health and Safety inspection was conducted on July 19, 2019 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-410-A-3 Description: Based on record review and interview, the lockdown procedures were not practiced annually. Evidence: Lockdown drill was not documented and the provider confirmed it was not completed. Plan of Correction: The lockdown drill is practiced twice a year, it was last practiced on May 3, 2019.
