Comments:

An announced initial inspection was conducted today from 11:20am-1:15pm. There were no children in care at the time. The home, 1 staff record, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There was a supply of books, toys, and materials for the children. Description:	Based on observation, the provider does not have a working battery-operated weather band radio. Evidence: The radio presented was not a weather band radio.
Plan of Correction:	I will buy a new radio.
Standard #:	22VAC40-111-940-B
Description:	Based on observation, the provider did not have an operable thermometer available to monitor the refrigerator and freezer compartment temperatures . Evidence: The provider could not present an operable thermometer available to monitor the refrigerator and freezer temperatures.
Plan of Correction:	I will buy a new thermometer.
Standard #:	22VAC40-111-240-A
Description:	Based on observation, all equipment in the home were not kept in safe condition. Evidence: The manufacture date of the crib is not decipherable therefore cannot be deemed in compliance with crib standards.
Plan of Correction:	I will call and get the manufacture date of the crib.

Description:	Based on record review, the provider failed to have the fireplace inspected annually. Evidence: The fireplace has not been inspected yearly.
