Comments:

An unannounced renewal inspection was conducted July 31 2019 between 9:30am and 11:30am. There were 6 children in the direct care of the provider and an assistant. Children were observed working on puzzles, being diapered, washing hands, and eating lunch. The day care space has age appropriate materials for the children in care. Standard #:	22VAC40-111-60-A
Description:	Based on record review, 1 of the 6 children in care did not have documentation on site of a child record. Evidence: Child # 4 record only contains proof of identification. Record is missing parent forms (liability insurance form, medication administration form, information for parents form, and the emergency preparedness and response plan form), child record documentation, immunization record, and physical examination record.
Plan of Correction:	I will tell the parents and give them the forms this evening.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, 2 of 6 children's records reviewed did not contain all of the required documentation. Evidence: Child #1 record is missing complete emergency contact information. Child #2 record is missing complete emergency contact documentation and who and when the original proof of identification was viewed,
Plan of Correction:	I will get the information from the parents.
Standard #:	22VAC40-111-70-A
Description:	Based on record review, 3 of 6 children's records are missing completed documentation that parents were provided all of the required information in writing prior to the child's first date of attendance. Evidence: Child #4, #5, and #6 records do not contain documentation that the parents received the liability insurance documentation, emergency preparedness and response plan, administration of medication form, and the information for parents form.
Plan of Correction:	I will get it printed out and give it to the parents this evening.
Standard #:	22VAC40-111-90-A
Description:	Based on record review, 2 of 6 children's records reviewed did not contain documentation that the provider obtained documentation that the child had been adequately immunized before the child entered into care. Evidence: Child #4 is in attendance, but does not have immunization documentation. Child #5 is in attendance and does not have documentation of immunizations.
Plan of Correction:	The parents are aware and will remind them this evening.
Standard #:	22VAC40-111-90-C
Description:	Based on record review, 1 of 6 child records reviewed did not contain documentation that the provider obtained additional immunizations every 6 months for a child under 2 years of age. Evidence: Child #2, aged 20 months, last documented immunization record is dated 1/2/2019

Standard #:	22VAC40-111-180-A
Description:	Based on record review, 1 of 2 staffing records reviewed did not contain documentation that the provider obtained a current Report of Tuberculosis Screening form every two years from the date of the first screening for each caregiver. Evidence: Staff #1 last documented Tuberculosis report is dated 07/06/2017
Plan of Correction:	I will call the doctor as soon as possible this week.
