Comments:
An unannounced renewal inspection was completed on 08/21/2019. There were 6 children, ages 23 months-3 years, totaling 13 points, 1 provider and 1 assistant present during the inspection. The inspector reviewed 5 children's records, one provider record, 2 assistant records and one pet record during the inspection. The provider stated that medication is not administered. Children were observed in the following activities: during learning time, during television time, in large motor play, having lunch and going down for nap. The inspector reviewed the updated emergency preparedness plans, emergency supplies and parent policies during the inspection. The inspector discussed the following with the staff: subsidy requirements, the renewal process, annual training and safe sleep. Standard #: 22VAC40-111-770
Description: Based on observations and discussion, the provider failed to ensure that a battery-operated flashlight shall be kept in a designated area and be available to caregivers at all times. Evidence: There was not a battery-operated flashlight available during the 08/21/2019 inspection.

Plan of Correction: A new flashlight will be bought.

Standard #: 22VAC40-111-60-B
Description: Based on record review and discussion, the provider failed to ensure that children's records shall contain all of the elements as required by the standards. Evidence: 1. The record for Child #1 did not contain the following information: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (the address for the second designated person was incomplete) and the immunization records for the child as required by 22VAC40-111-90. 2. The record for Child #2 did not contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; the addresses for both designated people were incomplete. 3. The record for Child #4 did not contain the following information: name, office address and telephone number of the child's physician, the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (the address for the second designated person was incomplete), the name and policy number of the child's medical insurance, the immunization records for the child as required by 22VAC40-111-90 and the name and address of previously attended child day care. 4. The record for Child #5 did not contain the following information: name, office address and telephone number of the child's physician, the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (the address for the second designated person was incomplete), the name and policy number of the child's medical insurance, the immunization records for the child as required by 22VAC40-111-100 and the name and address of previously attended child day care.

Plan of Correction: The provider will get the missing information and place it in the file.

Standard #: 22VAC40-111-140-B-3
Description: Based on record review and discussion, the provider failed to ensure that a current certification in CPR was obtained within the past two years. Evidence: 1. The most recent CPR certification in the record for Provider #1 expired on 05/20/2019. 2. The most recent CPR certification in the record for Assistant #1 expired on 05/20/2019. 3. The most recent CPR certification in the record for Assistant #2 expired on 05/20/2019.

Plan of Correction: A class has been scheduled.

Standard #: 22VAC40-111-140-B-4
Description: Based on record review and discussion, the provider failed to ensure that a current certification in First-Aid was obtained within the past two years. Evidence: 1. The most recent first-aid certification in the record for Provider #1 expired on 05/20/2019. 2. The most recent first-aid certification in the record for Assistant #1 expired on 05/20/2019. 3. The most recent first-aid certification in the record for Assistant #2 expired on 05/20/2019.

Plan of Correction: A class has been scheduled.

Standard #: 22VAC40-111-280
Description: Based on observations and discussion, the provider failed to ensure that potentially poisonous substances, such as cleaning agents and disinfectants, shall be stored away from food in areas inaccessible to children. Evidence: There was a gallon of Clorox under the unlocked bathroom sink that was accessible to children.

Plan of Correction: The Clorox was moved to a location that was not accessible.

Standard #: 22VAC40-111-320-B
Description: Based on observations and discussion, the provider failed to ensure that child-resistant protective covers shall be installed on all unused electrical outlets. Evidence: There was an uncovered unused electrical outlet in the room used by the children.

Plan of Correction: The outlet was covered during the inspection and will remain covered.

Standard #: 22VAC40-111-530-G
Description: Based on observations, record review and discussion, the provider failed to ensure that children under two years of age shall not use pillows or filled comforters. Evidence: Child #5 was observed sleeping with a pillow. Child #5 was determined to be 23 months old.

Plan of Correction: The pillow was removed and Child #5 will not be allowed to use a pillow until they turn 2.
