Comments:
A Subsidy Health and Safety inspection was conducted on Wednesday, August 28, 2019 from approximately 9:30 AM until 11:30 AM to review supplemental health and safety requirements. There were 11 non-resident children and one resident child in attendance being cared for by two caregivers. Based on the children?s ages there was a total of 31 points. The children were observed playing with toys, watching children?s programs on T.V., singing songs, infants being held, rocked and fed and taking bathroom breaks and washing hands. Five children?s records and two caregivers? records were reviewed during the inspection. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-170-B-12
Description: Based on the review of five children's records and interview, the caregiver did not ensure a written care plan was obtained for each child with a diagnosed food allergy, including instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for Child #4 indicated the child has a food allergy. A written care plan with the required information was not located in the record. 2. Caregiver #1 stated she was aware of the food allergy for Child #4 and a written care plan had not been obtained as of 8/28/19.

Plan of Correction: I will give the allergy care plan form that was given to me during the inspection and request the family has the child's doctor complete it.

Standard #: 22VAC40-665-230-E-1
Description: Based on documentation review and interview, the provider did not ensure all caregivers have current certification in cardiopulmonary resuscitation (CPR). Evidence: The record for Caregiver #1 contained a cardiopulmonary resuscitation (CPR) that expired on 8/12/17. Caregiver #1 stated she was not aware the cardiopulmonary resuscitation (CPR) training had expired.

Plan of Correction: I will contact the trainer and schedule a class.

Description: Based on documentation review and interview, the provider did not ensure all caregivers have current certification in first aid. Evidence: The record for Caregiver #1 contained a first aid card that expired on 8/12/17. Caregiver #1 stated she was not aware the first aid training had expired.
