Technical Assistance:
Discussed that following forms are not needed in the children records:
Information and Agreement, dated 6/99. 3 pages
Medication Consent form (MAT) is only completed if you are administering medication. This form is not needed if using the State's Medication Authorization form.
Provided a copy of the form, Authorization to apply non prescription topical skin products.

Comments:
An unannounced renewal inspection was conducted today between the hours of 10:30 am through 12:00. There were a total of 9 children (26 points) with 3 caregivers. A total of 5 children's files and 6 staff/household member's files were reviewed. The children were engaged in the following: outside play, toy play, coloring lunch, bathroom breaks,and hand washing. The Provider was not available this date due to a family emergency, the violations were reviewed with the substitute provider. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-170-A
Description: Based on a review of caregiver records, the Provider has not obtained a Tuberculosis (TB) screening for each caregiver.
Evidence: Caregiver #1, employment date 10/2018 does not have documentation of TB screening.

Plan of Correction: We will get the TB screening.

Standard #: 22VAC40-111-180-A
Description: Based on a review of household records, the Provider has not updated a TB screening for a household member.
Evidence: Household member #2 does not have a current TB screening. Last screening dated 6/5/17.

Plan of Correction: She will get an updated TB.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of caregiver records, the Provider has not obtained a Central Registry within 30 days of employment for each caregiver.
Evidence: Caregiver #1, date of hire 10/2018 does not have documentation of a central registry.

Plan of Correction: Caregiver is a sub and was called in today due to a family emergency. She will complete the central registry search form immediately.
