Discussion was held on the topics of current enrollment, current staffing, and review of training hours. Provider was able to complete a total of 20 hours of annual training for 2019.
Forms were left on site with regards to the renewal (FDH Renewal Application).
Staff forms were also left on site with regards to hiring a new assistant:
- Assistant/Substitute Provider Record
- 2 blank caregiver reference forms
- A record of annual training form
- An information guide on required background information
- A copy of a Central Registry Release of Information form with instructions
- A Sworn Statement or Affirmation for Child Day Programs form
- A report of tuberculosis screening form
The following inspection took place between the hours of 9:30 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Arlington, Virginia. At the start of this unannounced renewal inspection there were a total of 5 children (15 points) with 1 caregiver. A total of 5 children?s files, 1 staff file, and 1 household member?s file was reviewed. The children were engaged in the following: toy play, music, diapering, infant feeding, nap time, and other organized activities. Standard #: 22VAC40-111-830-B Description: Based on review, shelter-in-place procedures were not practiced twice a year.
1. On the date of inspection (08/30/2019), the posted drill log was reviewed. While a evacuation drill was conducted for each month in 2019, thus far there has been no record of a shelter-in-place drill.
2. This posting was on the main bulletin board within the child care area of the Family Day Home.
Plan of Correction: Shelter-in-place drills shall be conducted and recorded twice a year starting next month.
