Standard #: 22VAC40-111-200-A Description: Based on a review of caregiver records, the Provider did not orient the assistant provider by the end of their first week of assuming job responsibilities. Evidence: 1. Caregiver #2 (doh: 5/16/2019) did not have documentation of an orientation on file. Plan of Correction: I will give the orientation to the assistant provider and place in her file.
Standard #: 22VAC40-111-280 Description: Based on observation, potentially poisonous substances were accessible to children. Evidence: 1. In the bathroom, the cabinet under the sink did not have a lock on it and contained cleaning supplies. 2. There were 4 sunscreens that stated "keep out of reach of children" on the back and were stored in the children's bins that were accessible to the children. Plan of Correction: Corrected during inspection.
