Discussed renewal documents and safe sleep.

Comments:

An unannounced renewal inspection was conducted today from 10:15 am-1:00pm. There were 3 children (infants-22 months) supervised by 2 direct care staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in individual, playing outside, and eating lunch. Diapering and hand-washing procedures were also observed. There were books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in CPR and First Aid. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #:	22VAC40-111-60-B
Description:	Based on record review, the provider did not maintain required information in the children's records. Evidence: 1. Child #1 and #3 were missing address of parent's employment. 2. Child #3 was missing address for doctor's office. 3. Child #1, #2, and #3 did not have completed documentation for proof of age and identity.
Plan of Correction:	I will look at missing information and get from the parents today.
Standard #:	22VAC40-111-240-A
Description:	Based on observation, the provider did not ensure areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition. Evidence: 1. A hole in the wall located behind a door, the size of a door knob.
Plan of Correction:	I will fix today.
Standard #:	22VAC40-111-320-B
Description:	Based on observation, the provider did not have child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: 1. Two outlets were covered with master tape and one outlet did not have protective cover.
Plan of Correction:	I will get covers today.

Description:	Based on record review, the provider did not have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures. Evidence: 1. The provider did not complete two shelter-in-place procedures for more than a year.
Plan of Correction:	I will began completing two every year.
