63.2 Child abuse and neglect

Comments:
An announced initial inspection was conducted with the provider beginning at 9:01 AM and ending at 11:30 AM. There were four children in the care of the provider. The children's ages were 17 months, 2, 3, and 3 years old for a total of 9 points. The children were observed playing with blocks and toys. The children were also observed coloring letter-themed worksheets. Four children's records were reviewed. Three household records were reviewed. Four pet records were reviewed and in compliance. The first aid kit, flashlight, and weather-band radio were observed and in compliance. There were no medications on site to be reviewed. The most recent emergency evacuation drill was documented as 8/27/19.

Standard #: 22VAC40-111-60-B
Description: Based on a review of four children's records, not all of the children's records were complete. Evidence: Child A's record was missing the name and policy number of the child's medical insurance and name, address and telephone number of two emergency contacts if the parent cannot be reached.

Plan of Correction: The provider stated that she would obtain the missing information from the parent.

Standard #: 22VAC40-111-460-C
Description: Based on a review of four pet records, not all of the pets in the home had current rabies vaccinations. Evidence: Pet A and pet B's rabies vaccinations expired 6/9/19.

Plan of Correction: The pets will be taken to the vet for an update vaccination as soon as possible.

Standard #: 22VAC40-191-40-D-1-a
Description: Based on a review of documentation, not all household members had documentation of the sworn statement or affirmation and central registry background checks. Evidence: Household member A's record was missing a signed sworn statement and documentation of a central registry background check.

Plan of Correction: The provider stated that the central registry background check was requested and the sworn statement form was signed. The documentation was unable to be located during the inspection. The provider stated that she would obtain the missing documentation and place it in the record.
