An unannounced monitoring inspection was conducted from 10:30am-12:00pm. There were 5 children (20 points) in the direct care of the Provider. A sample of 3 children's records, 1 Caregiver record, evacuation drills, the first aid kit, emergency materials, and the day care space were reviewed. The children were observed doing tummy time, being fed bottles, and playing with toys. Any violations are listed on the Violation Notice. Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, the Provider did not obtain a Tuberculosis screening at least every two years from the date of the first screening.
1. Staff #1's last TB test was dated 8/14/2017.
Plan of Correction: I will have it corrected by this Monday.
Standard #: 22VAC40-111-570-A Description: Based on observation, the Provider did not ensure that a caregiver does not exceed 16 points.
1. The Provider was present with 5 children for a total of 18 points without a second caregiver available.
Plan of Correction: My Assistant quit without notice at the last minute, but one child will be leaving.
