- Technical Assistance:
An unannounced renewal inspection was conducted on Tuesday, September 17, 2019 to determine the provider's compliance with licensing standards. The inspection was initiated at approximately 10:45am and concluded at 12:55pm. There were five children present totaling 14 points in the care of one provider and one assistant. Upon arrival, the children were finishing up with circle time and were observed as they transitioned to watch a children's television show. The children were also observed playing with toys and interacting with the provider and assistant throughout the inspection. Later, the children were observed taking bathroom breaks, washing hands, eating lunch, and preparing for rest. The menu was posted and today's lunch consisted of: milk, hot dogs, cornbread, and pineapple. The areas for childcare, bathroom, kitchen, indoor and outdoor play area were inspected. The required postings were reviewed. The first aid kit and non-emergency supplies were inspected and found to be in compliance. Five children's records, two household members' records, and two providers' records were reviewed. Transportation is provided and one van was inspected today.

Standard #: 22VAC40-111-90-A Description: Based on a review of five children's records and interview on 09/17/2019, the provider did not obtain documentation that one child had been adequately immunized before the child may attend the family day home.
Evidence: 1) The initial immunization documentation in the file for Child #5, enrolled on 12/19/2018, was dated 12/26/2018. 2) During interview, Provider #1 reported there was no previous immunization documentation at the family day home for Child #5.
Plan of Correction: Corrected. Per the Provider: In the future, the provider will ensure immunizations are provided prior to the first day of attendance.
Standard #: 22VAC40-111-170-A Description: Based on a review of two adult household members' records on 09/17/2019, the provider did not obtain documentation of a tuberculosis (TB) test or screening for one adult household member.
Evidence: 1) The record for Household Member #2, move-in date of 07/29/2019, did not contain documentation of a tuberculosis test or screening.
Plan of Correction: Per the Provider: The TB test will be taken and results provided to the licensing inspector by next week.
Standard #: 22VAC40-191-40-D-4-a Description: Based on a review of two adult household members' records and interview on 09/17/2019, the provider did not ensure that one adult household member completed a sworn disclosure statement or obtained a central registry finding within 30 days of moving into the family day home.
Evidence: 1) The sworn disclosure statement on file for Household Member #2, move-in date of 07/29/2019, was dated 09/17/2019. 2) The record for Household Member #2 did not contain the results of a central registry finding. 3) During interview, Provider #1 reported that the central registry search had not been requested to date.
Plan of Correction: Per the Provider: The central registry search will be requested on 09/17/2019. The results will be emailed to the licensing inspector when they are received. In the future, the background checks will be obtained within the required time frames.
