Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, September 20, 2019. The on-site inspection was from 10:45 a.m. to 1:00 p.m. On this date, at the time of arrival, there was one child and one resident child in the care of the provider and an assistant (4 points). There was a total of one child enrolled in the program. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The record of one child was reviewed. There was no medication being administered or on-site, which is the policy of the provider. The records for the provider and one assistant were reviewed. All food was provided by the parents. The violation from the previous inspection was reviewed for compliance. Some items reviewed/observed included: required posted information, documentation of evacuation drills, injury reports, emergency supplies and equipment, water temperature, temperature of the fridge and freezer, free play and large motor activities on the playground, and dismissal procedures.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review of one child's record and verification with the provider, the child's record did not contain documentation that a physical examination had been obtained from a physician.

Evidence:
1. The record for Child #1 did not contain a physical examination report. Date of enrollment for Child #1 was 08/16/2019.
2. The provider confirmed that a physical examination had not been obtained.

Plan of Correction: The provider talked with the parent at pick-up time and a physical examination will be obtained as soon as possible.
