Evidence:
1. Child #4's record did not contain documentation of either parent's work telephone number, full addresses for either of the two required emergency contact persons; and parent's signed acknowledgement of the receipt of the information regarding policies for the administration of medications, and provisions of the family day home's emergency preparedness and response plan.
2. Child #5's record did not include documentation of a complete address for the second emergency contact person, the medical insurance policy number, and parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70.
3. Child #8's record did not include documentation of a complete address for the first emergency contact person.
4. Child #10's record did not include documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70 and provisions of the family day home's emergency preparedness and response plan.Plan of Correction: I will get all missing information from parents.
