Comments:
An unannounced monitoring inspection was conducted today with the substitute provider, who was present with two children ( 3 months and 22 months of age).The infant was in the swing while the toddler was engaged in free play.
Two children and caregiver's files were reviewed today. The substitute provider reported that there were no medication for any of the children present and there have been no injuries/accidents since the last inspection. Violations:
Standard #: 22VAC40-111-830-C
Description: Based on interview, the caregiver could not provide any documentation showing emergency evacuation and shelter-in-place drills.

Plan of Correction: Evacuation drills are done monthly. Shelter in place was conducted 4/19.

Standard #: 22VAC40-111-100-A
Description: Based on a review of two children's files and interview with Person A, it was determined that one child, child B enrolled on 3/19/18 and to date did not have a physical on file. Physicals are required within 30 days of attendance.

Plan of Correction: The parents will supply a copy of child's current physical on 10/24/19

Standard #: 22VAC40-111-50-A
Description: Based on observation of the attendance log, it was determined that there were two children ( 3 month and 22 months of age) present however the written record had no children as being present in the home.

Plan of Correction: Blank forms were placed on the board for attendance. Provider will write attendance as children arrive.

Standard #: 22VAC40-111-60-A
Description: Based on a review of two children's files and an interview with Person A, it was determined that one child, Infant A did not have a second emergency contact listed. Infant A enrolled on 9/23/19 and contacts are required at the time of enrollment.

Plan of Correction: After reviewing the child's record, there clearly was two emergency contact information. Form is included to show documentation.

Description: Based on a review of two caregiver's files ( Person A and B) it was determined that Person B's CPR certification expired on 10/21/19.

Plan of Correction: A copy of current CPR is provided for review, dated 10/5/19.

Standard #: 22VAC40-111-140-B-4
Description: Based on a review of two caregiver's files ( Person A and B) it was determined that Person B's First Aid certification expired on 10/21/19.

Plan of Correction: A copy of current First Aid is provided for review, dated 10/5/19.

Standard #: 22VAC40-111-140-D
Description: Based on interviews of two caregiver's, it was determined that the substitute provider (Person A) is not recording and signing the time of arrival and departures each day the substitute provider is working in the home for more than two hours without the provider being present.
Evidence:
One caregiver reported that the substitute provider is in the home alone with children from approximately 7 am until 3 pm daily. Another caregiver reported that the substitute provider is alone providing daycare to children 1 to 2 times and week from approximately 12 noon until 4: 30 pm.

Plan of Correction: The substitute provider assist in the care of the children daily with the provider. There are some days that I have to conduct PD visits at random times but it is not everyday, however the visits can sometimes exceed 2 hours.

Standard #: 22VAC40-111-200-A
Description: Based on a review of two caregiver's files, it was determined that there was no documentation that Person A has orientation training by the end of their first week of assuming job responsibilities. Person A reported that she has been working in the licensed home for years.

Plan of Correction: Person A has completed all initial teacher and background checks required documentation is provided.

Standard #: 22VAC40-111-530-F
Description: Based on observation, it was determined that an infant's crib contained soft bedding.
Evidence:
Infant A's ( 3 months of age) crib contained a filled blanket and two stuffed toys. Additionally, laying over the crib was a filled fabric blanket belonging to another child.

Plan of Correction: The soft bedding provided by the parents has been removed from the crib.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation, it was determined that the most recently issued license was not posted.

Plan of Correction: License has been placed on the board as you enter the daycare.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, it was determined that the findings of the most recent inspection was not posted.

Plan of Correction: The most recent inspection was tacked on the board at the entrance of the day care dated 4/27/19.
