Discussion was held on the topics of current enrollment, current staffing, and children files. Due dates for upcoming and current background checks was also discussed.
The following inspection took place between the hours of 9:30 am through 12 pm (noon). Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling Park, Virginia. At the start of this unannounced monitoring inspection there were a total of 5 children observed (10 points) with 1 caregiver. A total of 5 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: learning, music, song, dance, games, dress up, bathroom breaks, hand washing, lunch, and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 5 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to a child's attendance or within 30 days after the first day of attendance.
1. On the date of inspection (10/25/2019), documentation of a physical examination for Child #1 was not available for review.
2. The start date for Child #1 was listed as - 07/05/2017.
3. Child #1 was in attendance on the date of inspection.
Plan of Correction: A current physical shall be obtained for Child #1 and placed on file.
Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 5 children files indicated that not all of the required information was contained within each child's record.
1. On the date of inspection (10/25/2019), the policy number for the listed medical insurance for Child #5 was not available for review.
2. The start date for Child #5 was listed as - 01/05/2015.
Plan of Correction: This missing information shall be obtained and placed on file.
Standard #: 22VAC40-111-180-A Description: Based on review, 1 of 2 staff files indicated that the Provider did not obtain for each caregiver a current Report of Tuberculosis (TB) Screening form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent documentation on file.
1. On the date of inspection (10/25/2019), the most recent TB documentation on file for Staff #2 was dated - 03/06/2017.
2. Staff #2 was not present on the date of inspection.
Plan of Correction: An updated TB test shall be obtained and placed on file for Staff #2.
Standard #: 22VAC40-191-40-D-5-a Description: Based on review, 1 of 3 household member files indicated that a person living in the home of a licensed Family Day Home (FDH) Provider does not have a Child Protective Services central registry check (CPS check) within 30 days of a person in the home becoming 14 years old.
1. On the date of inspection (10/25/2019), a CPS check was not available for review for Household Member #2.
2. Based on information on file, Household Member #2 is between the age of 14 through 17.
Plan of Correction: A request for a search of the central registry shall be submitted for household member #2.
