Technical Assistance:
Discussion was held on the topic of new assistants, field trips and food brought from home.

Comments:
An unannounced monitoring visit was conducted today between the hours of approximately 11:30 am to 1:30 pm. There were 3 children (10 points) with the provider. A sample of 4 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: nap, toy play and snack. Records, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. There was no medication at site today. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not maintain a complete record for each enrolled child.
Evidence:
1) The records for Child #1 and Child #4 did not contain a signature for the policy of the family day home for the administration of medication.
2) The record for child #4 did not contain an address for the second emergency contact and information on allergies.

Plan of Correction: I will ask the parents to provide the information.

Standard #: 22VAC40-111-320-G
Description: Based on document review and interview, the family day home fireplace was not inspected annually.
Evidence:
1) Documentation in the family day home showed the last date of inspection was 06/30/2018.
2) The provider stated that she forgot to have it inspected.

Plan of Correction: I will call today to make the appointment.
