Comments:
An announced inspection was conducted today from approximately 10am-11:50am. There were 2 children (5 points) in the direct care and supervision of 1 caregiver. A sample of 2 children?s records, 1 caregiver's record and 3 household member's records were reviewed. The children were observed engaged with caregiver, engaged with inspector, and playing outside. Standard #: 22VAC40-111-70-A
Description: Based on record review, before the child's first day of attendance , parents were not provided in writing required information.

Evidence:
1. Child A began care 03/21/2017 and their parents had not received the following required documents: liability insurance waiver, walking field trip permission, medication administration waiver, and provisions of emergency preparedness.
2. Child B began care 06/22/2017 and their parents had not received the following required documents: liability insurance waiver, walking field trip permission, and medication administration waiver.

Plan of Correction: I will complete this weekend.
