An unannounced monitoring visit was conducted today. Four children (9 points) were present with two caregivers which is within ratios for supervision. Observed free play, reading and working with the alphabet, singing and manipulatives. The provider participates in Early Head Start and USDA food program. Lunch served today is chicken soup with veggies and rice, fruit and milk. No medication was reported to be on-site today. Evidence:
The Provider's TB report expired 3/13/2019 and the Assistant's expired 5/22/2019Plan of Correction: New screenings will be obtained for both caregivers.
