Technical Assistance:
Technical assistance was provided regarding proof of identification documentation and injury reports.

Comments:
An unannounced monitoring inspection was conducted between the hours of 11:05am through 1:25pm. There were a total of 12 children present ranging in ages 3 years through 9 years (16 points) with the Provider and 1 assistant. A total of 6 children's files, 2 staff files, and 1 household member's files were reviewed. The children were engaged in the following: coloring, reading books, playing outside, eating lunch brought from home, and engaging in various Montessori activities. Positive interactions between caregivers and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, attendance, and emergency procedures and supplies were completed today. The provider advised there was no medication on site today. The provider and assistant are both current in CPR/First Aid Training. There was an abundant supply of age appropriate materials and toys. Standard #: 22VAC40-111-830-A
Description: Based on review of the emergency drill log, emergency evacuation procedures have not been practiced monthly.

Evidence:
1. There was no documentation of an evacuation drill in July 2019 and August 2019.
2. The provider stated she did not do a fire drill in July and August.

Plan of Correction: We have had a drill every month since then and we will conduct a fire drill every calendar month.

Standard #: 22VAC40-111-60-B
Description: Based on review of 6 children's records, 4 children's records did not contain all required information.

Evidence:
1. Child #1's record was missing the annual review of emergency contact information. The last review of emergency contact information was completed 5/20/18.
2. Child #4's record was missing the annual review of emergency contact information. The last review of emergency contact information was completed 11/19/18.
3. Child #5's record was missing the annual review of emergency contact information. The last review of emergency contact information was completed 6/12/18.
4. Child #6's record was missing the annual review of emergency contact information. The last review of emergency contact information was completed 6/12/18.

Plan of Correction: We will have all parents review emergency contact information and sign and date.
