Technical Assistance:
Discussed and explained licensing procedures and reiterated that only 4 children could be in care without a license.

Comments:
An announced initial inspection was conducted today to determine compliance with licensing standards. The inspection was initiated at 10:15am and concluded at 12:45pm. There were 5 children (19 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. The physical plant, emergency supplies and procedures were inspected. Required documents were posted and cribs were inspected. Children were observed playing freely, eating lunch and napping. Today's lunch consisted of veggie pizza, mixed vegetables, apples, and milk. Hand washing procedures were also observed. The home was found to be clean and organized, and well supplied with toys and equipment for the children. The backyard offers outdoor play space for the children. Standard #: 22VAC40-111-770
Description: Based on review of emergency supplies, emergency supplies were incomplete.

Evidence:
A portable battery-operated weather band radio was not available during the inspection.

Plan of Correction: We have already ordered a weather band radio.

Standard #: 22VAC40-111-30-A
Description: Based on observation and interview, the provider did not meet the requirements specified in 22 VAC-80-60.B.3 , General Procedures and Information for Licensure.

Evidence:
The provider had 5 children in care during the inspection, they were caring for more than 4 children prior to the license being obtained. They did not ensure compliance with the standards issued by the department and with relevant federal, state or local laws, and other relevant regulations.

Plan of Correction: I will talk to the children's families to ask them to wait until I receive the license.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child#4's record contains an immunization record that was last updated on 5/8/2019.

Plan of Correction: I will ask the parents to bring me an updated copy of the immunization record.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, the provider did not obtain a child protective services central registry check for a staff member within 30 days of employment.

Evidence:
Staff#2's record (DOH:10/23/2019) does not contain a child protective services central registry check result.

Plan of Correction: I submitted the request to Richmond, we are waiting for results. I will check with the agency to find out the status of the search.
