Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 9 children files indicated that not all of the required documentation was complete and on file.
Evidence -
1. On the date of inspection (12/06/2019), a completed Medication Policy Form for Child #9 was not completed or signed by the parent of the child.
2. When reviewed, this form was blank on the signature page.Plan of Correction: This form shall be completed and then signed by the parent of Child #9.
Standard #: 22VAC40-111-210-A Description: Based on review, 2 of 2 staff files indicated that both caregivers did not obtain a minimum of sixteen (16) clock hours each of training annually in areas relevant to their job responsibilities.
Evidence -
1. On the date of inspection (12/06/2019), Staff #1 and Staff #2 each completed a total of 12 hours of annual training for 2019.
2. This total was confirm by the Provider on the date of inspection.Plan of Correction: A total of 16 hours each shall be completed by the end of the 2020 annual training period.
