Comments:
An unannounced monitoring inspection was conducted from 10:30am-12:15pm. There were 5 children (11 points) in the direct care and supervision of two caregivers. A sample size of 3 children's records, 2 caregiver/household member records, the outdoor play area, and the daycare space were reviewed. The children were observed playing outside on ride-on toys,l listening to songs, washing hands, and eating lunch (butternut squash soup, peaches, and milk), and watching an age appropriate show. Description: Based on a review of children's records, 1 out of 3 records was incomplete.

Evidence:
1. Child #1 was missing 2 of 2 required addresses for emergency contacts.

Plan of Correction: I will ask the parents for the addresses today.
