Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:50am am through 12:30pm. There were a total of 6 children (15 points) observed with 2 caregivers. A total of 5 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, puzzles, manipulatives, and lunch. Description: Based on children's record review, the provider has not updated the emergency contact information annually.

Plan of Correction: I will have the parents to review the forms

Standard #: 22VAC40-111-60-B
Description: Based on children's record review, a child's record did not contain the required information.
Evidence: Child #1, first date of attendance, 08/19/2019, record did not contain a parent's work phone number and medical insurance name and policy number.

Plan of Correction: I will ask the parent for the information

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances are found in an area accessible to children.
Evidence: The inspector observed disinfecting sprays and wipes located under the sink in a bathroom accessible to children. The cabinet drawer and doors had a safety lock, but was easily opened by the inspectors.

Plan of Correction: I will replace the locks on the bathroom cabinet and drawer.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of caregiver records, the provider has not obtained Central Registry results by end of 30th day of employment.
Evidence: Caregiver #1, date of hire, 04/01/2019, does not have documentation of a Central registry results.

Plan of Correction: Caregiver will get an ID number and the form will be submitted
