Comments:
An unannounced renewal inspection was conducted today between the hours of 2:10 pm through 4:45 pm with the provider on site. There were 12 children (24 points) with the provider and one assistant. A sample of 5 children's records and 4 caregiver/household member's records were reviewed. The children were observed during nap time, having snack, completing puzzles and playing with toys. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review for 2 of 5 children.

Evidence:
1. Child #1 began attendance on 1/7/19. The emergency information for Child #1 was last reviewed by parents on 12/1/18.
2. Child #2 began attendance on 2/5/18. The emergency information for Child #2 was last reviewed by parents on 12/5/18.

Plan of Correction: I will have them review and sign the information today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 5 children's records did not contain all required information.

Evidence:
The record for Child #1 did not contain documentation of the telephone number of the father's place of employment.

Plan of Correction: I will ask the parent for the information today at pick-up.

Standard #: 22VAC40-111-210-A
Description: Based on record review, 1 of 3 caregivers did not obtain 16 clock hours of training annually.

Evidence:
The record for Caregiver #1 (start date 11/27/18) did not contain documentation of training completed.

Plan of Correction: Trainings were completed. I will print and place in file.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review and caregiver interview, one household member did not have documentation of a current Central Registry search.

Evidence:
The most recent Central Registry search available for review in the record for Household Member #1 was dated 3/25/14. Provider stated a Central Registry search was mailed on 1/7/2020 once provider realized the record was out of compliance.

Plan of Correction: The central registry search was mailed on 1/7/2020. Provider will follow up once results are obtained.
