An unannounced monitoring inspection was conducted today. There are 5 children present (12 points) with one caregiver at LI arrival. A second caregiver arrived within 5 minutes.3 adult and 5 children's records were reviewed. See violation notice for further information. Children were observed playing quietly during today's inspection. Emergency escape drills, first aid supplies, fridge/freezer temperature are all in compliance. A subsidy Health and Safety Inspection (SHSI) was also conducted today.
Standard #: 22VAC40-111-60-A Description: Based on a review of records, one child's record was not maintained as up-to-date.
Child 2's record was signed 1/4/19 (past one year).
Plan of Correction: I will have the parent sign the form within the next 3 business days.
Standard #: 22VAC40-111-70-A Description: Based on a review of records, the parents have not been provided in writing with the notification of a pet in the home.

5 of 5 were missing notification of a pet in the home. (Children 1,2,3,4,5)
Plan of Correction: I will do an addendum to the children's records.
Standard #: 22VAC40-111-130 Description: Based on statement by the caregiver, the caregiver does not speak, read or write in English.
The caregiver present at LI arrival, stated she does not speak English. (Staff A)
Plan of Correction: I will either acquire a new assistant or have her enroll in ESL classes.
