An unannounced monitoring inspection was conducted today. There are 6 children present (14 points) with two caregivers. 2 adult and 6 children's records were reviewed. See violation notice for missing information. Children were observed finishing lunch, washing hands, during a diaper change, and resting quietly. Emergency evacuation drills, fridge/freezer temperature, menu, and emergency supplies are all in compliance.

Evidence:

*3 of 6 records reviewed were missing the insurance policy number. (Children 4.5.6)
*1 of 6 records was missing the first date of attendance. (Child #3)Plan of Correction: The parent was contacted for the insurance policy information.
The first date of attendance was completed during the inspection.
Standard #: 22VAC40-111-180-A Description: Based on a review of records, two adult records do not have current documentation of a subsequent Tuberculosis screening obtained every two years.
Evidence:
Adult A's TB documentation is dated 8/23/17 (expired).Plan of Correction: I am going to go and get it done.
