- Technical Assistance:
Technical assistance today included crib safety, child-proofing all areas that children walk past, practicing with the weather band radio, storage of first aid supplies, children's handwashing, use of push pins in walls.
An unannounced renewal inspection was conducted today from 10:15 am - 1 pm. There were 7 children present with the provider and assistant for 21 points. The provider and assistant are registered to take Phase 2 training this month. I observed children singing, playing with toys, eating snack, attempting to go outdoors but finding heavy rain, involved in a music and movement activity upstairs, eating lunch and beginning nap. Standard #: 22VAC40-111-920 Description: Based on observation during today's inspection, the food served for morning snack and lunch did not match with the posted menu. Plan of Correction: We will work on the menus.
Standard #: 22VAC40-111-960-E Description: Based on observation during today's inspection, none of the infant bottles were labeled as required.
Four baby bottles given to infants and toddlers today did not have labels.
Plan of Correction: We know them but we will make sure they are always labeled with name and date.
Standard #: 22VAC40-111-240-A Description: Based on observation during today's inspection, areas of the family day home were not maintained in a clean/safe/operable manner.
1. A panel of wood indoors next to the front door was loose, with a long screw back accessible. There were several feet of plastic colored industrial telephone wire coming through the panel, leaving it accessible at child height.
2. A gate at the bottom of the stairs in the children's area was attached with loose string on one side and not attached on the other side today.
3. There were loose threads accessible in many areas of the carpeted stairs
4. A square table top was stored behind a much smaller plastic shelf in the children's play area, and a child was able to slide the table top out, letting it fall.
Plan of Correction: 1 During the inspection, we put duct tape around the wire and took out the screw and loose panel.
2 The gate will be replaced.
3 We will cut the threads.
4 We will put the table in another place when it is not being used.
Standard #: 22VAC40-111-280 Description: Based on observation during today's inspection, some hazardous materials were found to be in unlocked areas at child height.
The unlocked closet on the stairs contained a large bottle of cleaner. Children were observed walking by as they went to upstairs play areas today. It was also accessible due to the broken gate by the stairs.
Plan of Correction: We will get a lock.
Standard #: 22VAC40-111-290 Description: Based on observation during today's inspection, children walked through the kitchen where a knife was observed in a drawer with a broken latch. Plan of Correction: It will be moved until the drawer lock is fixed.
Standard #: 22VAC40-111-430-B Description: Based on observation during the inspection, a pressure mounted gate was leaning against an opening at the bottom of the stairs.Gates at the stairs must be securely attached, not pressure mounted. Plan of Correction: We will get a new gate.
Standard #: 22VAC40-111-480-D Description: Based on observation during today's inspection, two cribs were observed not to be used as the manufacturer intended.
Both cribs present today each had two mattresses in each crib. The crib was designed for one mattress.
Plan of Correction: Removed during inspection.
