Comments:
An unannounced monitoring inspection was conducted today, January 21, 2020 to determine the provider's compliance with family day home licensing standards. The inspection was initiated at 11:00 am and concluded at 2:00 pm. Upon the inspector's arrival, six family day home children were in care with three caregivers for a total value of thirteen points. Children were working on printing letters and coloring. Later children danced to "Baby Shark" and read books. Lunch time was observed with children having bathroom breaks and washing their hands before sitting down to lunch. Today's lunch consisted of chicken nuggets, carrots, pears, bread and milk. Nap time was also observed with the sound of soft rain being played in the background. The caregivers interacted with the children in a positive manner. Areas used for child care were inspected along with a walk through of the entire home. The family day home has age appropriate materials and supplies. The family day home had the following required information posted: the license, most recent inspection notice, menu, emergency numbers, and attendance sheet. The first aid kit and non-medical emergency supplies were checked and found complete for the home and the vehicle. The inspector interacted with the caregivers and children in care.

Six children's records and the four caregivers records were reviewed along with the record of one household member.
An evacuation drill was documented on 12/10/2019. The two required shelter-in-place drills for 2019 were completed on 04/11/2019 and 06/10/2019. A lock down drill was conducted on 05/13/2019.

Per the provider, medication is not being administered.
The caregivers have current CPR & First aid certification.
Transportation is provided to and from school.

Violations:
Standard #: 22VAC40-111-140-D
Description: Based on interview, the provider did not maintain a record of the time of arrival and departure each day the substitute provider works.

Evidence:

1. The provider identified caregiver #1 as the substitute provider.
2. The provider stated caregiver #1 had worked in August of 2019 as a substitute provider while the provider was in the hospital for approximately seven days.
3. The provider stated she did not have documentation of the time of arrival and departure for the days caregiver #1 worked as the substitute provider.

Plan of Correction: Per the provider: I will print out a time sheet when the substitute is working.

Standard #: 22VAC40-191-60-B
Description: Based on review of caregiver records, the provider did not ensure an employee or volunteer of a licensed or registered child welfare agency or of a family day home approved by a family day system was employed or provide volunteer service before the agency home had the person's completed sworn statement or affirmation.

Evidence:

1. Caregiver #1 (hired 08/19/2019) who was on site and working, did not have a completed sworn statement on record.
2. The sworn statement in caregiver #1's record was not dated.

Plan of Correction: Per the provider: I will make sure sworn statement are completed and dated. Corrected while inspector was on site.
