Standard #: 22VAC40-111-700-C Description: Based on direct observation, the provider did not return medication, in which the authorization expired, within 14 days of the expiration of the authorization.
Evidence: A medication on site had a parent authorization dated 9/9/2019. Parent authorizations are only allowed for ten business days.
Plan of Correction: I will have the parents get the doctor to complete the correct form.
Standard #: 22VAC40-111-830-A Description: Based on observation, the provider did not ensure that emergency evacuation procedures were conducted monthly.

Evidence: Household member #1 record does not have documentation that a central registry search was completed. The provider stated that the search was submitted more than two months ago and that she will follow up with the appropriate agency.
Plan of Correction: Will follow up with the agency to get the forms as soon as possible.
