Comments:
A Subsidy Health and Safety inspection was conducted on January 30, 2020 to review supplemental health and safety requirements. The inspection was initiated at 12:45 pm and concluded at 3:00 pm. Upon the inspector's arrival four children were in care with one caregiver for a total of eight points. Children were observed singing songs with the caregiver. Later children played with toys, puzzles, in the playhouse and watched a video.Hand washing was conducted after toileting. Today's lunch consisted of fish, greens, bananas, spoon bread and milk. The main area used for child care along with a walk through of the entire home was conducted. The family day home is equipped with supplies and age appropriate materials. The following required information was posted; license, most recent inspection notice, emergency numbers, and various additional parent information. The first aid kit and non-medical emergency supplies were checked and found complete. An evacuation drill was documented on 01/23/2020. The two required shelter-in-place drills for 2019 were completed on 07/29/2019 and 11/22/2019. A lock down drill was documented on 11/22/2019. The inspector interacted with the caregiver and children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Four children's records and two caregiver's records were reviewed.
Per the caregiver: Currently there is no medication being administered. No transportation provided.

Standard #: 22VAC40-665-170-B-12
Description: Based on review of children's records, the provider did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:

1. Child #3, enrolled 11/15/2018, with a diagnosed food allergy did not have a written care plan that include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
2. A care plan for child #3 was on file however it was completed by the provider and parent and not the physician as required.

Plan of Correction: The provider called the parent while the inspector was on site requesting the parent obtain a written care plan from the physician. Per the provider: I will have the parent bring a written care plan from the physician for the record.
