Technical Assistance:
Discussed the following with the Provider:
- requirements and the process for moving the FDH to another house.
- timeframe for completing repeat background checks (5 years)

Comments:
An unannounced focused monitoring inspection was completed today. Today's inspection is being completed due to the FDH having been placed on a 6 month Provisional license. The physical plant, 5 caregiver records, 5 children's records, caregiver training, child nutrition, children's activities and evacuation drills were inspected between 11:45am- 1:10pm. There were 11 children, totaling 41 points, directly supervised by 4 staff. There are no caregivers certified in MAT (medication administration training) and there are no children with any medication needs at this time. Children were observed enjoying free play with toys while interacting with the caregivers, listening to story time and taking a nap . For lunch, the children were served grilled chicken, cauliflower rice, tator tots and milk. Description: Based on observation, there was a cleaning agent stored in an area accessible to the children in care.
Evidence
1. In the bathroom, there was a bottle of cleaning wipes on the sink.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-80-120-A-4
Description: Based on observations, the Provider failed to comply with the terms of the license.
Evidence
1. The FDH is licensed to care for up to 9 children at any one time. However, there were 11 children in care today.

Plan of Correction: The Provider requested and was recently approved to increase the capacity of the FDH through Arlington County. He emailed this approval documentation and a request to increase the capacity of his license to the Licensing Inspector during the inspection. The Licensing Inspector will process the modification request.
