Comments:

An unannounced monitoring inspection was conducted today from 11:50am-1:30pm. There were 9 children (24 points) directly supervised by 2 caregivers, the provider arrived at the Family Day Home approximately 15 minutes after the inspector arrived. The home, 3 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely eating lunch and napping. Diapering procedures were also observed. There was an adequate supply of books, toys, and materials for the children. Description:	Based on observation, infant bottles were not labeled with the date. Evidence: Three infant bottles were labeled with the child's name but did not have the date on them.

Standard #:	22VAC40-111-60-B
Description:	Based on record review, children's records did not contain all the required information. Evidence: Child#2's record did not contain the parents' place of employment name or address, and the address for both emergency contacts.

Standard #:	22VAC40-111-90-C
Description:	Based on record review, the Family Day Home did not have documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child#1's record did not contain documentation of updated immunization records. The most current immunization record on file was dated 1/28/2019.

Description:	Based on observation, a caregiver did not wash their hands with liquid soap and warm running water after diapering a child. Evidence: Caregiver#2 changed four diapers during the inspection, the caregiver did not wash their hands after completing each diaper change. A total of four consecutive diaper changes were observed and neither included hand washing before or after each diaper change.

Description:	Based on observation, electrical outlets were not covered with protective covers. Evidence: Two unused electrical outlets were observed without a protective cover in the programming area. One outlet was observed without a protective cover behind the television console, the other outlet was observed without a protective cover on the wall near the door that leads to the storage room.
