Comments:
An unannounced monitoring inspection was conducted today between the hours of 11:30 am through 1:30 pm with the provider on site. There were 12 children (30 points) with the provider and two assistants. A sample of 5 children's files and 6 caregiver/household member's files were reviewed. The children were observed engaging in free play with toys, singing songs, reciting words in Spanish, and during naptime. The physical space, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. There were no medications on site. Standard #: 22VAC40-111-780-B
Description: Based on record review, 1 of 5 records did not contain documentation that the provider reviewed with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

Evidence:
Child #1 began attending the family day home on 10/23/17. The last review of emergency information for Child #1 signed by parents was dated 10/20/18.

Plan of Correction: I will do it today at pick-up.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Evidence:
Child #2 began attending the family day home on 10/30/17. The physical exam for Child #2 was dated 2/16/18.

Plan of Correction: I will make sure to get the physical within 30 days.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that one child has been adequately immunized before the child began attending the family day home.

Evidence:
Child #2 began attending the family day home on 10/30/17. The first record of immunizations documented in the record for Child #2 was dated 2/16/18.

Plan of Correction: I will make sure to have immunizations before they start.
