Technical Assistance:
Technical assistance was provided on correcting violations before a license will be issued.

Comments:
An unannounced focus inspection was conducted today from 9:00am-11:00am. There were 2 children (7 points) supervised by 1 direct caregiver. The home, 2 caregiver records, 2 children?s records, 2 household member?s records, evacuation drills, and policies were inspected. There were no medications on site today. Children were observed engaging freely and eating snack. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-111-120-B
Description: Based on review of two caregiver records, one is incomplete.
Evidence:
Staff #1 has an incomplete record: it is missing application, emergency contact information, and age verification.

Plan of Correction: She is my daughter and doesn't work often, I will get the information and put it in the file.

Standard #: 22VAC40-111-200-A
Description: Based on review of two caregiver records, one is incomplete.
Evidence:
Staff #1 does not have documentation of completing an orientation within their first week of assuming job responsibilities.

Plan of Correction: She doesn't work that much, and is my daughter, I will give her an orientation and put it in the record.

Description: Based on observation, potentially poisonous substances are accessible to children.
Evidence:
Potentially poisonous substances were accessible to children:
laundry room was unlocked, hand sanitizer-keep out reach of children was laying on floor, shampoos and other personal items were in unlocked shower area.

Plan of Correction: I usually lock those rooms

Description: Based on observation, not all electrical outlets are covered with a protective cover.
Evidence:
Electrical outlets in the children's play area by the TV area are exposed to children and not covered.

Plan of Correction: I will cover them all with protective covers

Evidence:
The is no documentation of the findings of a chimney/fireplace inspection.

Plan of Correction: I called them, they will send me a copy of the results.

Description: Based on observation, an accordion expansion gate id being used as a protective barrier at stair opening.
Evidence:
An accordion expansion gate is being used as a protective barrier at stair opening of the child care area.

Plan of Correction: We replaced it, the new one didn't lock correctly so I took it down. A new one will be up this week.

Standard #: 22VAC40-111-680-A
Description: Based on observation, caregiver did not wash her hand before serving snack.
Evidence:
Caregiver #2 gave snack to the children without washing her hands first.

Plan of Correction: I will make sure to always wash my hands.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregiver did not ensure children's hands were washed with liquid soap and warm running water before eating snack.
Evidence:
Children sat at table to eat snack without washing their hands to eat.

Plan of Correction: I will make sure their hands are washed before eating.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface is not being cleaned and sanitized according to the licensing standards.
Evidence:
The diaper changing table is not being cleaned with a solution of soap and water, patted dry, then sprayed with a solution of bleach and water after each diaper change.

Plan of Correction: I will begin using the proper procedures.
