Technical Assistance:

Technical assistance today included: new gardening area outdoors and strengthening accessible wooden posts near the stairs; record keeping, physical environment, balancing part time enrolled children; CPR/First Aid training. .

Comments:

An unannounced renewal inspection was conducted today from 10:15 am - 12:35 pm. There were 9 children present for 27 points with the provider and two assistants. I observed children playing outdoors, eating lunch, toothbrushing and preparing for naptime. There were no medications on -site today. Records were well-organized. Standard #:	22VAC40-111-100-A
Description:	Based on a review of children's records today, some children had a record of immunizations but no physical exam within 30 days of attendance. Evidence: Child #2, who began care 10/19; and Child #3, who began care 9/19, did not have documentation of a physical examination.
Plan of Correction:	The parents will be reminded to bring this in.
Standard #:	22VAC40-111-60-B
Description:	Based on a review of children's records today, there was some information that was not completed as required. Evidence: There were no health insurance policy numbers listed for Child # 1, 2, 4, 5. The emergency contact information for Child # 3 # 4 did not have complete addresses as required.
Plan of Correction:	We will talk to the parents and get this information.
Standard #:	22VAC40-111-240-A
Description:	Based on observation during today's inspection, there were some outdoor areas that were not maintained in a safe condition. Evidence Outdoors: The slide structure had a crack in the side of the plastic slide chute. There were some areas where landscape material was coming through the mulch, creating a tripping hazard.
Plan of Correction:	The slide will be removed or replaced, but for now the slide will not be used. The mulch will be swept from the side of the fence to cover the landscape material, and more mulch will be ordered.
Standard #:	22VAC40-111-250-E
Description:	Based on observation and staff confirmation, some balloons were present today. Evidence: Three air balloons filled with hearts were decorating the children's area, and one came down during the visit. Additionally, bag of unblown balloons was present on the top of shelf.
Plan of Correction:	We will not use this type of ballloon

Description:	Based on observation during today's inspection, there were some hazardous materials accessible. Evidence: The cabinet above the toilet was unlatched, making cleaners accessible.
Plan of Correction:	We will make sure to always close the child-proof lock on that cabinet.
Standard #:	63.2-1720.1-B-3
Description:	Based on a review of staff records today, the provider did not have the results of a background check before a staff member began employment. Evidence: Assistant # 2 began work 2/4/2020, but there was no completed Search of the Child Registry results present before beginning work.
Plan of Correction:	It has been mailed out.The other background check was returned. I will send it to you as soon as I receive it.
