Comments:
An unannounced monitoring inspection was conducted on February 10, 2020, from approximately 9:42am until 11:00am to determine the provider?s compliance with licensing standards. There were five children in attendance being cared for by one caregiver. Based on the children?s ages there were a total of 12 points. Upon arrival, the children were observed in the designated area for childcare playing with toys and listening to music on a tablet. The caregiver was observed having positive interactions with the children. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys and materials to meet the needs of the children.
During the inspection, the licensing inspector interacted with the provider and children when appropriate. The bathroom, kitchen, and designated childcare areas were all inspected during today?s inspection. The inspector observed the required postings. The first aid kit and non-emergency supplies, fire and shelter-in-place drills were inspected. Five children?s records, one caregiver record, and three household member?s records were reviewed during the inspection.
Per the provider medication is not administered and transportation is not provided at this time. The most recent evacuation drill was documented on 01/14/2020. The most recent shelter-in-place drill was documented on 01/14/2020.
The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Description: Description: Based on record review, the provider did not maintain an up-to-date record for at the family day home for two of five records reviewed.
Evidence: 1. The record of Child #3 (DOE 6/12/17) did not contain documentation of the name and policy number of the child's medical insurance.
2. The record of Child #5 (DOE 9/2/19) did not contain documentation of the policy number of the child's medical insurance.

Description: Description: Based on record review, the provider did not ensure that before a child may attend the family day home, the provider obtain documentation that the child has been adequately immunized.
Evidence: The record of Child #4 (DOE 9/3/19) contained documentation of immunizations dated 10/8/19, which exceeds the child's first date of attendance.

Standard #: 22VAC40-111-540-A
Description: Description: Based on observation on February 10, 2020, the provider did not ensure that infant carrier seats, swings, strollers, feeding or activity tables, and high chairs were used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.
Evidence: Child #4 was observed sleeping in an infant bouncy seat. The child was not securely fastened into the seat.
