Reminded Provider again that the MAT medication form is not needed.
The medication authorization form only needs to be completed if the child has a medication that needs to be administered.
The 3 page Information and Agreement form does not need to be completed, it has the same information as the required "Child Record" form.

An unannounced monitoring inspection was conducted today between the hours of 8:40 am through 10:00. There were a total of 10 children (27 points) observed with 3 caregivers. A total of 6 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: toy play, bottles, legos, diapering, hand washing and coloring.
