A Subsidy Health and Safety inspection was conducted on 02/25/2020 to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-160-B Description: Based on documentation review, the provider did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1) The attendance record for the family day home documents the children that have arrived by highlighting their names in yellow. There is no indicator as to when the departure of each child occurs.Plan of Correction: I will ensure arrival and departure times for each child is logged.
Standard #: 22VAC40-665-230-B Description: Based on record review, the provider failed to ensure all caregivers completed the Virginia Preservice Training for Child Care Staff.
Evidence:
1) The records for Staff 1 and Staff 2 did not contain documentation that they had completed the Virginia Preservice Training for Child Care Staff.Plan of Correction: Proof of completion of the VA Preservice training for Staff 1 and 2 will be collected and filed.
Standard #: 22VAC40-665-230-E-1 Description: Based on record review, the provider did not ensure that all caregivers held a current certification in cardiopulmonary resuscitation (CPR).
Evidence:
1) The records for Staff 1 and 2 did not contain a current certification in CPR. The most recent certifications were dated 09/16/2017 for Staff 1 and 09/09/2017 for Staff 2.Plan of Correction: Current CPR certification for Staff 1 and 2 will be collected and filed.
