Technical Assistance:
Technical assistance today included physical environment, on-line DSS training opportunities, record keeping, posted menu changes and infant sleeping arrangements.

Comments:
An unannounced renewal inspection took place today from 10 am - 12:15 pm. There were 9 children in care for 26 points with the provider and 2 assistants. I observed children busy with dress up clothes,painting a picture, playing outdoors, listening to a story and preparation for lunch. Violations:
Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records today, there was a parent agreement that was signed but did not have all required information.
Evidence:
The Medication Policy was signed by the parents of Child # 1 ,2 and 4, but the provider did not indicate the limits of the policy as required.

Plan of Correction: I will check the box and have the parents initial it.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records today, one child did not have documentation that he has been adequately immunized according to the requirements of the Code of Virginia and State Board of Health.
Evidence:
Child #4 had documentation of some immunizations, but no signature from the doctor or health department representative that stated he was current on immunizations for his age.

Plan of Correction: I will talk to the parents.

Standard #: 22VAC40-111-210-A
Description: Based on a review of staff records, there was not adequate annual training as required for one of the assistants.
Evidence:
Assistant # 1 did not have documentation of 16 hours of annual training as required.

Plan of Correction: She will take on-line classes.

Standard #: 22VAC40-111-240-A
Description: Based on observation during today's inspection, there were some areas and furnishings that were not in a safe, operational condition as required.
Evidence: A metal piece (from a screen door) extended about four inches at ankle height from the front door, causing a tripping hazard. The brown plastic playhouse in the yard was missing the roof and very shaky, causing instability. Exposed landscape material on the ground posed a tripping hazard.Two portable plastic slides were placed in areas where bare soil was observed in fall zones, and one slide was placed in an area that sloped downward, accelerating the downhill ride.
Indoors: There was a broken blind on the back of the front door. After removal, it left a space wide enough between the strings to pose a strangulation hazard

Plan of Correction: We will move the slides so they are in areas that have softer ground and cut back or cover the landscape fabric so it is not exposed. The metal piece on the door will be removed. The blinds will be removed and the door will be covered with something else for privacy.
