Comments:

An unannounced complaint investigation was conducted beginning 2/12/2020 and ending 3/6/2020. The Northern Licensing Office received the complaint on 2/11/2020 regarding care of children. Interviews were conducted and documentation was reviewed. The preponderance of evidence does not support the allegation and the complaint has been deemed not valid. Violations were found that were not related to the complaint. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Tuesday, March 24, 2020. Description:	Based on record review and provider interview, one assistant record did not contain all required information. Evidence: Staff #2 (Date of Hire: 2/3/20) did not have documentation that two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction:	I have now obtained references for this staff member. Moving forward, I will have references on all assistants upon hiring.

Description:	Based on interviews, the provider did not ensure compliance with the terms of the current license issued by the department. Evidence: The provider is currently operating outside the terms of her license by providing care beyond the licensed hours of operation. The provider's current licensed hours of operation are 6:00am through 6:30pm Monday through Friday. The provider stated she usually provides care for Child #1 (Date of Enrollment: 1/27/20) on Mondays and Wednesdays from 5:30am through 7:00pm and on Tuesdays from 5:30am through 9:30pm.
Plan of Correction:	I will stick with my regular hours for daycare of 6:00am through 6:30pm and I won't operate beyond those times.

Description:	Based on record review, the provider did not complete orientation with one assistant by the end of their first week of assuming job responsibilities. Evidence: Staff #2 (Date of Hire: 2/3/20) has not completed orientation.
Plan of Correction:	I have completed orientation and as of this day I will make sure I have everything done upon hiring new staff.

Description:	Based on record review, one staff record did not contain the results of a fingerprint based criminal history background check prior to first day of employment. Evidence: Staff #2 (Date of Hire: 2/3/20) did not have fingerprint based criminal history background check results on file.
Plan of Correction:	The staff has submitted to fingerprint testing and eligible results were received 2/18/20. Moving forward, I will have background check results on file before new staff begin working.
