Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was initiated and completed on 5/4/2020. The provider was contacted by phone to initiate the inspection. A remote visual inspection was conducted by phone in order to observe the home, areas designated for child care, and the required documentation.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. If you have any questions, please contact the licensing inspector at 804-662-9092.

Violations:
Standard #: 22VAC40-111-770
Description: Based on an interview, the provider did not ensure that the home is equipped with a working portable battery-operated weather band radio. Evidence: During an interview with the licensing inspector, the provider reported the home was not equipped with a weather band radio.

Plan of Correction: The provider will purchase a portable battery-operated weather band radio.

Standard #: 22VAC40-111-790
Description: Based on observation and interview, the provider did not ensure that the required emergency telephone numbers were posted in a visible area close to the telephone. Evidence: 1. The licensing inspector did not observe a phone number posted for the responsible person for emergency backup care; and did not see the number for 911 or local dial numbers for police, fire, and emergency medical responders. 2. The provider reported the numbers for the emergency backup person and 911 were not posted in the home.

Plan of Correction: The provider will post the required emergency phone numbers.

Standard #: 22VAC40-111-460-H
Description: Based on observation and interview, the provider did not ensure that psittancine birds (birds of the parrot family) shall not be in areas accessible to children. Evidence: 1. The licensing inspector observed a parakeet in a cage located in the back porch area of the home. The licensing inspector observed children's toys, a toy kitchen set, and children's chairs located on the back porch area of the home. 2. The provider reported the bird observed on the back porch is a parakeet. The provider reported children will use the back porch area during care for playing.

Plan of Correction: The provider will ensure that the parakeet will be moved from the porch during the hours that children are in care.

Standard #: 22VAC40-111-500-D
Description: Based on observation, the provider did not ensure that swings shall have flexible seats of rubber, canvas, or nylon. Evidence: The licensing inspector observed two swings with hard plastic seats, located on the play equipment in the outdoor play area of the home.

Plan of Correction: The provider will purchase two new swing seats that are made of flexible material to replace the hard plastic seats. As of today, the provider will ensure children do not use the swings with hard plastic seats by making them inaccessible to children until they can be replaced.

Description: Based on a review of one provider record, and an interview, the provider's record did not contain the Background checks that are required at the time of initial application. Evidence: 1. The licensing inspector observed that the record for Provider #1 did not contain a Central Registry search result or a Sworn Statement. 2. During an interview with the licensing inspector, the provider reported the Central Registry request had been sent on 1/20/20. The provider was unable to locate the results during the inspection. The provider reported no follow up contact had been made to obtain the results. 3. During an interview with the licensing inspector, the provider reported being unaware of the requirement for a Sworn Statement.

Plan of Correction: The provider will follow up by phone to obtain the Central Registry search results and follow their instructions. The provider will fill out the Sworn Statement form and submit this to the licensing inspector.

Standard #: 22VAC40-191-40-C-1-c
Description: Based on a review of one adult household member record, and an interview, the adult household member record did not contain the Background checks that are required at the time of initial application. Evidence: 1. The licensing inspector observed the record for Household Member #1 did not contain a Central Registry search result or a Sworn Statement. 2. During an interview with the licensing inspector, the provider reported the Central Registry search request had been sent on 1/20/20. The provider was unable to locate the results during the inspection. The provider reported no follow up contact had been made to obtain the results. 3. During an interview with the licensing inspector, the provider reported being unaware of the requirement for Household Member #1 to provide a Sworn Statement.

Plan of Correction: The provider will follow up by phone to obtain the Central Registry search results and follow their instructions. The provider will fill out the Sworn Statement form and submit this to the licensing inspector.
