63.2 Licensure and Registration Procedures
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/18/2020 and concluded on 05/21/2020. The program director was contacted by telephone to initiate the inspection. There were no children in care during the inspection; the facility is closed due to the pandemic. The inspector emailed the provider a list of items required to complete the inspection. The inspector review 2 children's records and 2 caregiver records during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Standard #: 22VAC40-111-60-B Description: Based on record review and discussion, the provider failed to ensure that each child's record shall contain all of the elements as required by the standards.
Evidence: The record for Child #1 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provisions of medical treatment on religious or other grounds or the names and addresses of previously attended child day care and schools.
Plan of Correction: The provider will call the parent and ask for the information.
