- Technical Assistance:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A focused monitoring inspection was initiated on May 22, 2020 and concluded on May 22, 2020. The provider was contacted by telephone to initiate the inspection. There were five children present and one staff. This visit focused on the main areas of previous non-compliance.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
Standard #: 22VAC40-111-60-A Description: Based on a review of two children's records, one record did not contain the required documentation. Evidence: 1. The record for Child #1 (start date 9/17/19) did not contain a medical insurance policy name or number. The record indicated that the child's insurance information would be available on 10/19/2019. Plan of Correction: The provider will have the parent update the child's record.
Standard #: 22VAC40-111-70-A Description: Based on a review of two children's records and an interview, the provider did not provide the required written information to parents prior to attendance for the children Evidence: 1. The records for Child #1 (start date 9/17/19) and Child #2 (start date 9/9/19) did not contain the following documentation: Information for Parents, Policy for the Administration of Medication, Liability Insurance Declaration, or the Provisions of the Home's Emergency Preparedness and Response Plan. 2. During an interview on 5/22/2020, Caregiver #1 reported being uncertain about the written information that is required to be provided to parents before a child's first day of attendance. Plan of Correction: The provider request that the licensing inspector email copies of the department's forms entitled "Liability Insurance Declaration", "Policy for the Administration of Medications", "Information for Parents", and "Provisions of the Emergency Preparedness and Response Plan." The provider ensure all children's files are updated with signed documentation that parents have received this information, and will ensure that the parents of all newly enrolled children are provided this information prior to their first day of attendance.
Standard #: 22VAC40-111-330-A Description: Based on an interview, the provider did not ensure that there was a landline telephone available, operable, and accessible that does not require electricity to operate. Evidence: 1. The provider stated the home did not have a landline phone that does not require electricity to operate. Plan of Correction: The provider states she is filling out an allowable variance request for this requirement.
