The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on May 29, 2020. The provider was contacted by telephone to initiate the inspection. There were no children present and (1) staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) staff records submitted by the family day home (FDH) to ensure documentation was complete.
Information gathered during the inspection determined a non-compliance with an applicable standard and was documented on the violation notice issued to the FDH.
If you should have any future questions, please call or send an e-mail to firstname.lastname@example.org. Thank you.
Standard #: 22VAC40-111-180-A Description: Based on review, 1 of 2 staff files indicated that the Provider did not obtain for each caregiver a current Report of Tuberculosis (TB) Screening form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most current documentation on file.
1. On the date of the previous inspection - 11/13/2019, the following information was recorded: Staff #1 - TB documentation dated.
2. The Provider confirmed (during an inspection interview conducted on 05/29/2020), that this is the most recent TB documentation on file for Staff #1.
Plan of Correction: Updated TB documentation shall be obtained and placed on file.
